About this service
The Street Outreach Team is a multi-disciplinary, multi-agency program that is centralized within Public Health, City Of Hamiltons Mental Health and Street Outreach Service
Outreach Workers work with individuals who are experiencing homelessness and have barriers to obtaining housing and supports within the community.
The Outreach team assists individuals in connecting to supports around health care, income, housing and other appropriate community support services.
The Street Outreach worker Provides a presence on the street, in shelters, and other community drop-ins and programs.
To Refer Someone For Outreach Services ...
The individual must currently be experiencing homelessness and may or may not be experiencing issues relating to mental health.
Referrals are made by calling IntAc. at (905) 528-0683
