About this service

The Shelter Health Network Hep-C Team consists of a team of highly specialized Hep-C professionals specifically dedicated to those living with or at risk of acquiring Hepatitis-C.

We are an outreach team. We meet clients/patients where they are at anywhere in the community (home, coffee shop, etc.). We have access to office space in our building to host one on one visit with clients, and would be open to seeing patients at our building, our yours (or anywhere else the patient feels comfortable).

The Shelter Health Network Hep-C Team provides support and comprehensive services to clients throughout the treatment process. Our services include outreach, assessment, referral, psycho-social counselling, peer support, education, awareness, medical assistance, and support throughout the treatment process. The Shelter Health Network Hep-C Team recognize harm reduction and best practice, focusing on healthy living and recovery.
