About this service

Since 1979, the expansion of home care services has been part of the fundamental orientation currently guiding the evolution of our health and social services system : the development of services IN the living environment is clearly what that people want.

The support and accompaniment Marc-Simon (SAMS) is therefore intended to be a personalized accompaniment and home support program that is well adapted to the needs of our members, considering the reality of today's PHAs.

GOALS

  • Support a greater number of members
  • To improve the overall health of our members
  • To plan the difficulties encountered in achieving a stable and satisfactory state of health
  • Prevent periods of disorganization and support the person during it
  • Ensure that each new case, where possible, is taken care of more quickly
  • Coordinate a strong and sustained social safety net for our members using community and public network service offerings
