About this service
The Interprofessional Primary Care Team at the Scarborough Centre for Healthy Communities is a comprehensive program designed to provide patients access to a coordinated team of healthcare professionals. The focus is on integrating primary care services to enhance patient experiences and address the needs of vulnerable patients with complex healthcare requirements. The team includes nurse practitioners, registered nurses, social workers, mental health case workers, harm reduction workers, registered dieticians, chiropodists, physiotherapists, and occupational therapists. Services offered include primary care, women's health, LGBT+ care, chronic disease management, harm reduction, nutritional counseling, and more. To receive services, patients need to be referred by a family doctor or nurse practitioner or can directly contact them if they do not have a family doctor. Eligibility requires the patient or referring provider to reside in Scarborough.
