About this service
Community Support is offered to anyone living with a life-limiting illness who is over 15 years old and lives in Etobicoke. You may refer yourself, or a family member/friend with their consent.
The Care Coordinator may suggest specific Hospice services that will provide immediate support. If the Dorothy Ley Hospice is unable to help directly, we will put you in touch with other community agencies that can meet your needs.
- Services in your home where you are most comfortable
- Care management to help you navigate the health care system and advocate on your behalf to access the services you need
- Day Program to provide social interaction for you and short-term respite for your caregiver
- Integrative Wellness programs to promote a sense of well being and quality of life
