About this service
Traverse Independence offers outreach services: our community facilitator works with you and your family in your home. The community facilitator is there to support you to establish the routines in your new place of residence. In some cases ongoing weekly support is needed on a long term basis. The community facilitator works with your community based rehab team to apply goals and strategies to daily living in the following areas of independent living.
Who can use it?
ABI outreach program the applicant must: participate in the development of their goals and work consistently on the completion of those goals. Reside in the region of waterloo or county of wellington. Reside in their own personal home including a retirement home. Applicants living in ltc facilities are not eligible.
