Kitchener Downtown Community Health Centre

Kitchener Downtown CHC provides primary health care to eligible Kitchener residents and facilitates access to a broad range of health and social services through referrals and community partnerships. Primary health care takes into account the social determinants of health and a range of services that include medical care, health promotion, illness prevention and community capacity building.

KDCHC is more than a medical centre. The health centre provides a range of programs and services that promote health and prevent illness. KDCHC believes that individual and community health is determined by health determinants and a number of factors including; Housing Employment & work environment Income Food access & security Social support systems Social inclusion/exclusion Education & literacy, including health literacy Environment Culture Gender To influence the health determinants, clients, community members and individuals are actively participating, discussing common concerns, and acting together to improve and promote their health and well-being.

Eligible kitchener residents and facilitates kitchener downtown chc focuses on clients who have barriers to accessing health care and often have health inequities. Priorities include isolated seniors, youth at risk, people with low incomes, new canadians and people who are homeless/homelessness at risk.

Kitchener Downtown Community Health Centre is open Monday 9:00am-5:00pm Tuesday 9:00am-5:00pm Wednesday 9:00am-8:00pm Thursday 9:00am-5:00pm Friday 8:00am-4:00pm.
