About this service
Intensive Case Coordination works to provide linkages and collaborations with other service providers to ensure continuity and coordination of services. Case coordination works in partnership with community organizations, primary care, mental health, addictions, housing providers, and all other service providers that assist in addressing the needs of this complex population. Many of the clients who require the support of intensive case coordination have significant and complex barriers. Intensive Case Coordination can assist individuals who have an ABI to learn new skills, can provide support and strategies to optimize their quality of life.Case Coordination and Occupational Therapy can provide functional support while waiting or are receiving Outreach and Transitional Living Services.
