AMHS-HPE - Bancroft - Mental Health Case Management is a program offered by the Canadian Mental Health Association (CMHA), Hastings Prince Edward Division. The service utilizes the Recovery Model and offers a range of mental health support services across Hastings and Prince Edward Counties. These include community-based support, individualized treatment planning, advocacy, and family education. Hospital liaison case management provides additional support for patients transitioning from psychiatric units to community living. Other services include the Peer Mentor program to assist with daily living skills, and emergency room diversion case management for addiction or mental health crises. Integrated case management for concurrent disorders is also available. The CMHA also provides short-term transitional mental health case management for high priority cases requiring swift response. All services aim to promote independence, wellness, personal growth, and community living.
