About this service
Our Outreach Programs allow us to meet with older adults in the community who are potentially at risk of losing their housing, or who may be experiencing a crisis that can impact their health, housing stability and overall well-being.
The goal of our Outreach Programs is to prevent homelessness, address gaps in services, and work with community/government partners using a wrap- around approach to care. This allows older adults to age in their own homes, maintain safe and adequate housing and feel supported in the community of their choice.
