If you are 65 years or older and experiencing dementia and other mental health and/or substance use concerns, our multi-disciplinary seniors mental health & substance use services teams can help. Our teams provide specialized assessments, treatment, case management and community support as well as caregiver support. Services may include: - mental health & competency assessments - medication administration, review & monitoring - care coordination & referrals - self-management supports - life skills education & support - family education & support
