Inter-Agency Care Team (IACT)
- Interdisciplinary team of clinical and non-clinical staff including social work, nurse practitioner, psychiatric nurse and outreach support workers.
- Provide mobile outreach support for people experiencing homelessness.
- Assistance with filling out forms and various paperwork such as PWD applications, SAFER, CPP, CPPD, OAS and income assistance applications.
- Service linkages and referrals to housing, treatment, income supports and a variety of other programs.
- Wrap around support for people who are vulnerable and not accessing traditional programs.
