About this service
The Follow-up Program provides a seamless transition of programming for our Residential clients who have worked with the Residential Case Manager during their stay. The Follow-up Program provides three months of Outreach supports without duplicating services, but building on the work done previously, providing supportive listening, safety strategies, ongoing abuse assessments as needed and maintenance of community resources already linked, assisting in further referrals as needed.
