SANCTUM 1.5 Sanctum 1.5 is a 10-bed prenatal care home which supports high-risk and HIV positive pregnant women at risk of having their infants apprehended at birth. Sanctums vision is to provide a community for individuals requiring supportive care to attain their optimum level of health, while maximizing their control and dignity. Incorporating existing community services in Sanctums safe and supportive environment, the prenatal home will work preventively to meet the health and social needs of these women more effectively and efficiently which will improve the health and wellbeing of mom and baby while also mitigating the risks associated with apprehensions of infants at birth. Through the collaboration of health and child welfare, Sanctum 1.5 will reduce the financial burden on the healthcare system and more importantly the long-term detrimental impacts on children involved with the foster care system. Sanctum 1.5 will achieve this by eliminating the current fragmented model of care provided to high risk and HIV positive prenatal mothers by incorporating the right services at the right time to allow for the necessary health and social interventions to have the most effective outcomes SANCTUM 1.5 WILL PROVIDE AND FACILITATE: A holistic home like environment for both mother and baby (Mothers can enter the program at any stage of their pregnancy and will be able to return to Sanctum 1.5 from hospital with their babies for an additional 3 months) Physician support (prenatal and pediatrics) EAT SLEEP CONSOLE (non-pharmacological approach to treating Neonatal Abstinence Syndrome in infants) Intensive family support Intensive HIV Case Management Medication management Connection with detox services Methadone assisted recovery Healthy Mother Healthy Baby Elder support Parenting skills and mentorship Peer support Life skills Access to housing supports Spiritual care Mental health and addiction outreach services Family planning Transitional planning and support (mothers and their children will have access to housing and support for an additional year post discharge from Sanctum 1.5) REFERRAL CRITERIA Women must be pregnant and any of the following: HIV positive or At risk of contracting HIV while pregnant (Injection drug use etc.) And At risk of having their child apprehended at birth (substance misuse, mental health, homeless, exposed to violence, other children in care etc.) Resident of Saskatchewan SINCE SANCTUM 1.5 OPENED IN 2018: The Sanctum 1.5 Model Sanctum 1.5 opened on October 1, 2018, a one of a kind wrap around support program for women who are pregnant as well as who are HIV positive or who are at risk of contracting HIV through injection drug use. This program simultaneously addresses the health, wellbeing and empowerment of high-risk pregnant women; the reduction of mother-to-infant vertical transmission of HIV; the health and bonding of newborns; and the systemic issues of children entering the foster care system. The program offers a supportive home for women and facilitates mother-infant bonding and positive parenting. As of May 2022, 92 mothers have successfully completed/graduated the program 93 babies have been prevented from entering foster care. In addition, mothers who have successfully graduated the program have had an additional 56 children returned to their care. A total of 149 children are now successfully within the care of their mothers, either through prevention or return from foster care. The Sanctum 1.5 model of care helped to achieve one of ten calls to action in the 2017 Saskatoon Health Region external review related to Saskatchewan women who reported cases of coerced sterilization. In March of 2020, the Canadian Institute for Health Research granted the Sanctum Care Group $1.3 million dollars to support evaluation and creation of an evidence-base that will support scale up and replication of Sanctum 1.5s model provincially, nationally , and globally due to overwhelming success of the program.
