About this service
This program provides emergency respite services for a period of up to fourteen (14) days to Windsor Essex County seniors age 65+ years; who are exhibiting a mental health problem(s) in maintaining their independence; requiring 24 hour respite supervision from Personal Support Health Workers as determined by the Emergency Department GEMs, Psychiatric Assessment Nurses, Physicians and Mental Health staff in the emergency department.
Objectives
- To support the senior's needs to return home with improved compliance in managing their own care; improved quality of life and independence
- To assess and optimize linkages between community and professional services, caregivers(s) and family; ensuring a wrap-around service model of care
- To provide family/caregiver relief
- To show evidence in the reduction of senior patient Emergency Department visits and hospital admissions thereby directly and indirectly improve access and reduce overcrowding in hospital emergencies
