About this service
Bereavement Care Coordinators assist clients who have experienced loss. The Bereavement Care Coordinator is responsible for the intake, assessment, and service provision for clients living and coping with grief and bereavement. The coordinator will be involved in case management and case consultations with other health professionals in Durham Region. The coordinator will ensure continuity of 1-1 programming and bereavement support group programming.
This program is based on a peer support model, which means that volunteers who are bereaved and further along in their grief journey, provide support to clients who are also bereaved.
