Transitional Care Services

What is Short-Term/Transitional Care? Our Specialized Community Services (SCS) team can facilitate the coordination of healthcare after a person is discharged from a hospital or healthcare facility. This care incorporates different types of therapy for healing and can be administered in your home, nursing homes, private care homes, long-term care facilities, or at one of our specialized community homes. SCS can also support short term placement for children and adults when there is an interruption to their permanent home and provide housing, support and other services are required. How can Short-Term/Transitional Care help my loved ones? If you or your loved one have recently been discharged from the hospital but still needs assistance with recovery and readapting to their home environment, our SCS team can ensure their safety and well-being. Once discharged from the hospital, our healthcare professionals will be there to help with the continued recovery journey. We will support your loved ones while they heal by assisting them in various ways including nursing, assistance with daily tasks, treatment of wounds, and medication management. Our SCS team places the person at the centre and develops an individualized care and support plan that is key to a better health outcome. Providing the individuals with the support needed to move from hospitals and other healthcare settings to assisted living residences, nursing homes, or back home. Short-term/transitional care is appropriate for: Individuals with developmental disabilities or require mental health support Individuals who no longer requiring hospital level of care Individuals undergoing cancer treatment Individuals requiring restorative or rehabilitative care to regain independence after an accident, trauma, an acute phase of illness, surgery Individuals requiring support to manage chronic or complex medical conditions Individuals with neurological conditions such as brain injury, stroke Children/youth in need of short term housing and care
