The Diabetes Strong Clinic is newcomer focused. Our team works diligently with each and everyone to ensure they are empowered to self-manage their blood sugars and receive appropriate referrals to best manage their health and thereby significantly reduce their risk of complications.
Access equity to quality diabetes care and prevention for those new to Canada is why we get up each day. Many of us are Canadians with recent migration histories ourselves. the care and is a inter-professional health care team of nurses, dietitians, certified diabetes educators, social workers and chiropodist, and physician..
