About this service

Our multi-disciplinary team of Dieticians, Diabetic Nurse Educators, Social Workers and an Outreach Worker provide a traditional, culturally-based program which targets Indigenous people across the lifespan. This program offers screening, intake, and health assessments for seniors in the community. The services provided by our team include: community workshops, community kitchens, screening access, intake and referrals to primary healthcare practitioners, and diabetic education.
