About this service
The Life Long Care Program (LLCP) ensures meeting individual needs through physical, mental, emotional and spiritual aspects of everyday life by providing support services to all urban Aboriginal clients, regardless of age, who are disabled, chronically ill, frail elderly and/or require acute/chronic continuum of care. The Life Long Care Program helps provide a wholistic approach to community support services, enhancing and improving the quality of life and living for the clientele and allowing them to live independently in their community. In addition, the Life Long Care Program strives to ensure quality of care and appropriate support systems for their Caregivers.
OBJECTIVE
To Support the Timmins Native Friendship Centre, to deliver the LLCP services in compliance with the Ministry of Health and Long-Term Care and Ontario Federation of Indian Friendships Centre policies.

Indigenous persons with disabilities, chronically ill, frail elderly or require acute/chronic care
