About this service
Our Diabetes Education Program team is comprised of a Dietitian and Diabetes Nurse Educator, who provide individualized and group counselling sessions, both within our Community Health Centre and through outreach clinics within Misiways catchment area. This program accepts referrals from patients and/or health care providers which makes access quick and easy.
Individual sessions include glucometer training, nutritional and weight loss counselling and individualized diabetes management plans. Counselling also includes sharing of knowledge to incorporate traditional diet choices.
Group sessions include Diabetes Sharing Circles, Meal Preparation Classes and Family Sharing Sessions.
