About this service
Passport is funded by the Ontario Government for the purpose of fostering independence, increasing participation in the community, promoting social inclusion, helping young people transition from school to the community and support families and caregivers. It is available to anyone who already qualifies for adult developmental services through the Ministry of Child, Community and Social services, including anyone 18 years of age and still in school. Passport Funds are able to be used for supports such as:
Community participation and Activities of Daily living
Paying a support worker to help with community participation and daily living activities
Caregiver respite
Person-directed planning that builds on the individuals interests and strengths, and identifies the supports needed to help the individual achieve their goals, and
Administration of Passport funding (up to 10% of Passport allocation).
