About this service
This vital program provides support to youth with an intellectual disability who are between the ages of 18 to 25, as they transition from school to independence. The goal of the program is to use a person-centred approach to support the young adult in building the confidence needed to make informed decisions, and to reach their independence in a healthy and productive way.
The Youth Transition Worker works collaboratively with the individual, their family and support networks to identify the goals the individual wishes to achieve. Subsequently, the Youth Transition Worker will support the individual in developing the tools and skills needed to reach their goals. Supports may include:
Training to take the town bus,
Financial literacy,
Healthy relationships curriculum,
Gaining skills to maintaining a household,
Guidance with volunteering opportunities, and
Opportunities to develop positive social relationships with peers.
