About this service
The AMHS - HPE - Trenton - Mental Health Case Management offers a range of services to individuals needing mental health support across Hastings and Prince Edward Counties. The services are centered around the Recovery Model, which promotes independence, wellness, personal growth, and community living. The key services include community-based service, individualized assessment and treatment planning, supportive counseling, service navigation, system advocacy, personal advocacy, family support and education, skills groups, public education, and consultation, relapse prevention, and crisis planning. They also provide specialized services for hospital discharges, including coordination with hospital staff for seamless transition, ongoing supportive counseling and support after discharge, and linkage to other programs and services. The program operates a Peer Mentor program that assists individuals with daily living tasks and a Transitional Case Management program for urgent, short-term needs. There's also a Concurrent Disorders program for those experiencing both addiction and mental health issues.
