About this service
The Adult Case Management (ACM) Program provides support and oversight of individuals who have transitioned to adult funded MCCSS supports and services but, continue to reside with an Outside Paid Resource (OPR) or within their natural family home. ACM provides ongoing case management of the individual by assisting in developing advocacy skills to establish personal goals and objectives as well as advocating for the individual and/or families when they are not able to do so themselves. These supports are provided in an efficient and effective manner to ensure that individuals are being provided with exceptional service within the adult developmental service sector.
