About this service
Chronic Disease Prevention and Management is focused on the prevention and control of various chronic diseases that commonly affect people over the age of 45. Such diseases are often linked to lifestyle choices, including diet, level of activity, and habits such as smoking. Common chronic diseases include heart disease, hypertension, type 2 diabetes, and chronic obstructive pulmonary disease (COPD). While most of these diseases are preventable, once they occur they can often only be managed, not cured. However, the quality of life can be significantly improved through proper management of these chronic ailments. The Central Hastings Family Health Team emphasizes the importance of empowering patients to take an active role in managing their health and offers related programs such as COPD & Smoking Cessation, Diabetes, and Cardiovascular Health. Practitioners work with a multidisciplinary team consisting of mental health counselors, registered dieticians, nurses, and family doctors.
