About this service
The Home 2 Stay program assists individuals 55 years of age and older who are at risk of homelessness remain housed or find alternative safe, sustainable housing.
Home 2 Stay provides intensive case management that works to stabilize clients housing by carefully assessing their risks and strengths, developing an individualized plan, monitoring progress, and making effective referrals to internal and community supports, advocacy and support with tribunal issues, and rapid rehousing.
Home 2 Stay follows individuals for 12-24 months to ensure they are successful in maintaining their housing.
