Two Rivers Family Health Team

Two Rivers Family Health Team is dedicated to delivering excellent, inter-professional health care to meet the diverse needs of our community. Offering a number of programs and services. In addition to program-based care, our patients receive clinic-based primary care from HealthLink Family Practice and Hespeler Medical Clinic.

The Two Rivers Family Health Team originally began with 12 physicians coming together in 2006 to provide after hours care to their patients. Today, the team has grown to 21 physicians working in collaboration with a wide range of allied healthcare professionals that include: nurse practitioners nurses pharmacists dietitians social workers occupational therapist physiotherapist psychologist psychiatrist The team is supplemented with medical office assistants, receptionists and administrative support staff at each site. As we have grown as a team, we have successfully launched several chronic disease management programs including but not limited to Diabetes Education, Newborn Support and Nutrition Education, to provide added benefits and enhanced care to our patients. In addition to program-based care, our patients receive clinic-based primary care from HealthLink Family Practice, and Hespeler Medical Clinic.
