About this service

The Transition Home provides safe and affordable housing to young people aged 16-24 who are looking for a home but are not ready to live on their own yet. The Transition Home Program helps to prepare you for the real world by teaching you the skills you will need to move on to more independent living. The home is staffed 24 hours a day with caring and supportive counselors who are available to help you with any issue you may have. When you move in, you are paired with a Residential Counselor who will work with you one-to-one to help you determine how you will reach your short- and long-term goals. They will also work with you on developing the life and social skills you require before you move on to more independent living or return home. Our counselors can help you explore your options and can also make referrals to Youthab's housing, mental health, and employment programs or to other community resources.
