About this service
Bancroft Community Family Health's Diabetes Education Program is designed to assist patients in comprehending and managing their diabetes and reducing their risk of diabetes-related complications. The program is open to self-referred individuals and primary care provider referrals aged 18 and up. It is delivered by a specialized team of nurses, dietitians, and foot care nurses. The program provides individual counseling on healthy diets, active lifestyles, blood glucose monitoring, and complication prevention. Additionally, it offers group classes, cooking classes, prenatal diabetes care, prediabetes education, and foot care services. The Bancroft Community Family Health Team has been providing collaborative, comprehensive healthcare to its community for over 35 years.
