CMHA - Colchester East Hants - Direct Service Programs

The Community Outreach Worker helps those in the Colchester-East Hants area who are not currently connected to community supports, by helping them navigate whats available and accessing community resources that are right for them.This service is unique in that it seeks to serve both Mental Health Consumers, as well as those who care for them.

Services include offering individual support and advocacy for those who might be having difficulties accessing benefits or communicating effectively with medical professionals, speaking to Community groups, individual caregiver/mental health consumer support (short term) and co-facilitating the Families Matter in Mental Health course.

The age range for those seeking service is 20+ years. Individuals can self-refer, be referred by community agencies and partners, or be referred through Mental Health and Addiction Services.
