Multi-disciplinary team responding to community citizens who are experiencing a mental health or psychosocial crisis. - The team provides: emergency room/hospital 1-1 assessment, outreach services or community visits prioritized by degree of urgency. - Crisis/warm call line operates 24 hours a day, 7 days a week, providing community citizens experiencing a mental health or psychosocial crisis and/or referral sources with: ongoing support, crisis intervention, consultation, linkages and education.
