You must be assessed for eligibility before you receive this service. Contact Contact Vancouver Community Central Intake at 604-263-7377 to learn more. If you have been in hospital and need home care, the VCH Transition Team staff in every Vancouver hospital will handle referrals from hospital to community services. If you are already receiving home care services and need additional help, you can contact your Community Health Centre directly.
