About this service
The Transitional Care Centre (TCC) and The Hub offer time-limited, transitional accommodation for patients discharged from St Pauls Hospital (SPH) who are in need of:
Short term shelter
Assistance that is not available at the time of discharge, and where this assistance would facilitate connection to community services
A safe alternative to the street, post-discharge, to bridge the individual to existing care and/or existing safe place
A safe alternative to the street in order to bridge the individual to a new safe place, to be obtained with assistance from community services or TCC/HUB staff, that is unavailable at the time of discharge
The TCC/HUB has no official capacity as a clinical treatment facility; rather its objective is to ensure individuals are given the opportunity to move into the next level of service.
