About this service
Enhance existing health services for patients 19+ who have 2 or more complex chronic diseases. Services are provided to patients, families and caregivers and include:
- System navigation
- Care coordination and shared care planning
- Interventions
- Education and self-management
Teams are attached to doctors' offices and clients are seen in offices and Interior Health sites; clients must be community mobile as home visiting is not provided.
