About this service

Individuals must be registered with the Fraser Health (FH) Palliative Care Program through a referral from a physician or health care professional. The Palliative Care Team will conduct an assessment and determine eligibility for palliative care services including admission into Hospice. Persons in care wishing to be admitted to Hospice must:

  • agree with the goal of hospice care
  • have a life expectancy of less than six months
  • cannot be cared for at home
  • require symptom management but not hospital care

Inquiries regarding eligibility and registration with the FH Palliative Care Program should be directed to the Abbotsford/Mission Palliative Consult Team Office at 604-870-7522.
