About this service
Provides assessment and consultation for seniors at risk for or currently experiencing adverse changes in cognition and/or mental health, which compromise function and quality of life. Provides consultation, assessment, and support for the treatment of adults 65 years of age and older with memory, cognitive, behavioral, and/or mental health concerns. Assessments may be completed at home, lodge, hospital, Designated Supportive Living, or Long Term Care settings. Education and information are provided to seniors, families, caregivers, and staff. Designed for seniors at risk for or currently experiencing adverse changes in cognition and/or mental health, which compromise function and quality of life.
