About this service
An individualized treatment program designed for seniors who have fallen, are at risk for falling, or are experiencing symptoms of functional decline.
This program is designed for community dwelling seniors who are at risk for falling or who have fallen and are experiencing symptoms of functional decline. Functional decline is often a non-specific mix of physical, psychological, social and functional manifestations. This may include difficulty with basic and instrumental activities of daily living, mobility, gait and balance.
