About this service
The Community Support Team provides community based services to adults with dual diagnosis (developmental disability and accompanying mental health diagnosis). We work as allies and believe that all individuals belong in the community regardless of the complexity of their support needs. We also offer specialized services for adults that are deaf and/or deaf blind. Support can range from 5 hours a week to 24 hours per day.
To be eligible for The Community Support Program the individual must have Persons with Developmental Disabilities (PDD) funding.
Referrals can be made by the individual, family, health care provider, PDD or supporting community members
