All applicants must meet the criteria for CLSD involvement. All requests for services, ongoing programmatic services and issues related to services are reviewed, discussed, and decided at the Admission Review and Transitions Committee.
All people requesting or using services provided by CPH must indicate a desire to receive services in accordance with the agency philosophy of care.
