• Person living with mental health problems
• Person living with problems of adaptation and social integration

• Person demonstrating a motivation to get involved in the program

Reference mode

• Visit or presentation of a case by a worker from the health network
• Evaluation Form

Means of intervention

• Preparation of a personalized action plan by the user and the worker, jointly
• Sustained supervision for the acquisition and/or strengthening of daily functional skills
• Individual weekly meetings focused on the objectives determined in the action plan
• Acquisition of social skills: hobbies, friends, activities, social norms
• Progress notes and evaluation of the individual's progress
• When indicated and available, use of a studio room for:
• verify and consolidate previous acquisitions
• learn new skills
• check the degree of comfort and autonomy in a lighter frame
• work more on socialization

Duration of stay

• From 3 to 6 months
