About this service
Program strives to empower and equip individuals, 16+ years, with severe and persistent mental illness to achieve both success and satisfaction in their chosen environment through a systematic client-driven rehabilitative process. The goal of the Program is to transition the client from extensive hospitalization to community living environment.
The program has an 8 bed capacity with a maximum length of stay of 2 years, providing individualized, structured programming that empowers and equips you with daily life skills that facilitate your return to the community; intensive skill development and support to increase your level of independence.
Your inter-professional healthcare team will consist of a Registered Nurse, Registered Practical Nurse, Occupational Therapist, Recreational Therapist, Psychiatric Attendants, and Psychiatrists, all partnered with HDGHs local Assertive Community Treatment (ACT) services.
