About this service
Htel-Dieu Grace Healthcares (HDGH) Dual Diagnosis Consultation program, located on the CPH floors of the Toldo Neurobehavioural Institute, is an outreach stabilization service for those individuals, 16 years of age or older, with a pre-existing Developmental Disability indicated by an IQ of 70 or less, and also have a Psychiatric Disorder.
What can I expect?
A two to three (2-3) hour assessment will take place to determine if you are eligible for Dual Diagnosis Consultation services. The assessment will be completed by a registered nurse and will take place at HDGH or potentially in your home.
Based on the information collected, an intervention plan will be created to monitor and stabilize your symptoms. A meeting will then be set up with you, your family or caregiver, the nurse, and the programs Psychiatrist.
Your healthcare team, along with your primary healthcare provider, and community organizations will work together to create your personalized treatment plan.

Individuals 16+, with developmental disability (IQ 70 or less) and also have a psychiatric disorder
