About this service
ICM promotes independence and improved quality of life with the primary focus on people with a serious mental illness through the coordination of appropriate services and the provision of ongoing support as needed using a recovery model focus. The direct involvement of the individual and the development of a supportive relationship between the community support worker and the individual are integral to developing personal recovery goals. Case management is a collaborative service consisting of interrelated processes to support individuals in their efforts to achieve optimal health and independence in a complex health, social, and physical environment. The overall goal of case management is to improve the individuals ability to function in the community.

People over the age of 16 primarily with a serious mental illness
