About this service
VON Durham has a special RN role, the Palliative Pain and Symptom Management Consultant (PPSMC), to provide consultation, education, mentorship for healthcare providers. PPSMC can support the continuity of care and linkages to palliative care resources.
The service assists health care providers in the delivery of palliative care & the interpretation & application of the Model to Guide Hospice Palliative Care using evidence-informed best practice. (Canadian Hospice Palliative Care Association, CHPCA).
The aim of the service is to ensure that anyone living with a life-limiting
illness has access to pain & symptom management expertise.
