About this service
VON Durham Site Home and Community Services (VON Durham) provides Hospice Palliative Care Services and is home to the SMART, WIN, and SELECT programs. We provide emotional, social, and spiritual support to individuals and families facing life-limiting diagnoses, advanced illness, death, and bereavement.
Our staff and trained volunteers work in partnership with community health care professionals and other service providers in an effort to enhance the quality of client care. Our staff and volunteers are also available to provide anticipatory grief and bereavement support to family members and friends, as well as advance care planning support.
