About this service
If you require health care or personal support in your home or community, we get you the care you need, when you need it. Through personal visits and regular check-ins, we help determine the right care and health supports for people and their families in every community across Ontario.
Many people are not aware of the options available to them when it becomes difficult to live independently at home. We understand the challenges a family faces when someone's health-care circumstances change. We can help you live at home for as long as possible and help you get back on track after a hospital stay, an illness or injury.
Our team of dedicated nurses, occupational therapists, social workers and other care professionals help people heal at home and live in their homes longer. As well, we help coordinate helpful home care services like assistance with bathing and dressing, meal delivery, transportation services and other activities of daily living.
