About this service
Primary health care is the first point of contact a person has with the health care system when they have a health problem or issue that is not an emergency. Our interprofessional team of physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians, and others work together to provide services and programs that meet the specific needs of our clients and community, including the diagnosis and treatment of health problems; promotion of good health; prevention of health problems; early intervention; and the management and care of ongoing and long-term health conditions. Primary care services include: - Scheduled medical appointments - Comprehensive assessment to determine resources to meet your needs - Referrals and follow-up of community resources - Medication review appointments and follow-up - Lab services - Home visits - On-call 24/7 - Same-day urgent care appointments - Cancer screening - Follow-up after an emergency department visit or hospital discharge
