About this service
Multi-Disciplinary Access To Care & Housing (MATCH) was founded in 2015, to provide ongoing intensive support to individuals referred from the Multi-Disciplinary Outreach Team (M-DOT) and Coordinated Access to Care for the Homeless (CATCH) teams who need continued involvement of a wrap-around team.
The two referring programs are both rapid-access, transitional programs while MATCH provides long term follow-up.
The MATCH team consists of Case managers, a part-time Registered Nurse (RN) and a part-time Psychiatrist, after-hours phone support is provided on evenings and weekends.
Emphasis is placed on making a good connection with the client before the transfer from the referring team is completed.
