About this service
Community Support is an intensive Case Management service aimed at helping adults recover from mental health challenges. Typically this is accomplished by meeting in a community setting with a Case Manager who helps with setting goals, finding resources and navigating the various systems (health, income, housing, etc.) in a persons life.
Who is eligible to apply for Community Support
at Toronto North Support Services?
To apply for this service you must be:
16 years of age or older, and
Living in the area bounded by Steeles Avenue, the Humber River, Eglinton Avenue and Victoria Park Avenue, and
Experiencing mental health issues that seriously impact your life
